Eye enucleation

Enucleation is indicated in blind, painful, or diseased eyes where vision cannot be restored
In this case, chronic inflammation following trauma led to irreversible ocular damage and secondary endophthalmitis
Surgical removal of the globe eliminated the source of pain and prevented spread of infection to surrounding tissues or the contralateral eye

Surgery

A 5-year-old female Domestic Shorthair cat was presented to the clinic with a chronically swollen and painful right eye.
The owner reported a history of ocular trauma from a cat fight approximately one month prior. Initial treatment with topical antibiotics from a local clinic provided only temporary relief.

  • Severe buphthalmos
  • Corneal opacity and ulceration
  • Purulent discharge with chemosis and conjunctival hyperemia
  • Absent menace and pupillary reflex
  • Signs of endophthalmitis (intraocular infection)

Clinical findings and ocular ultrasound, the cat was diagnosed with:

End-stage right eye (blind and painful) secondary to chronic trauma and endophthalmitis.

Transpalpebral Enucleation: it is used in

infected or neoplastic eyes, or when there is ocular surface contamination (ulcer, perforation, purulent discharge)

Steps :

  • Suture upper and lower eyelids together with simple continuous 3-0 nylon before dissection.
  • Eyelid Closure:
    • This seals the ocular surface to prevent orbital contamination.
  • Incision:
    • Make a full-thickness skin incision around the eyelid margins.
  • Dissection:
    • Proceed bluntly through subcutaneous tissue and orbicularis muscle to reach the orbital septum.
    • Dissect posteriorly around the closed globe.
  • Transection:
    • Identify and sever the extraocular muscles.
    • Transect the optic nerve as posteriorly as possible.
  • Hemostasis:
    • Achieved by pressure packing and ligation if needed.
  • Closure:
    • Close subcutaneous tissue with 3-0 absorbable sutures.
    • Skin closed routinely with 4-0 non-absorbable sutures.
